Community Counseling and Mediation (CCM), a Brooklyn-based social services organization, has just been awarded a federal grant from SAMHSA (Substance Abuse and Mental Health Services Administration) to carry out a “Minority AIDS Initiative Continuum of Care Pilot – Integration of HIV Prevention and Medical Care into Mental Health and Substance Abuse Treatment Programs targeting Racial/Ethnic Minority Populations at High Risk for Behavioral Health Disorders and HIV” in Brooklyn, NY.
In partnership with HEAT (Health & Education Alternatives for Teens), an affiliate of SUNY Downstate Medical Hospital located in Central Brooklyn, NY, CCM’s program will be providing behavioral health screening; primary substance abuse and HIV prevention; substance abuse, mental health, and co-occurring treatment; HIV and hepatitis screening and testing, and hepatitis B and C vaccination. 278 unduplicated clients annually in Central Brooklyn will receive holistic, culturally and linguistically sensitive counseling services that utilize the evidence-based theories of Motivational Interviewing, Cognitive Behavioral Therapy, and Personalized Cognitive Counseling.
The Program Coordinator will be responsible for the delivery of all program aspects, ensuring that the entire cohort of 278 clients are receiving comprehensive, integrated, co-located services that address their substance abuse, mental health, and health needs. The Program Coordinator will provide direct supervision to two (2) case managers, two (2) peer leaders, and two (2) outreach workers. S/he will meet regularly with subcontractors, including the HEAT Program/SUNY Downstate. S/he will identify areas of collaboration with other community agencies and non-profits that might work to enhance the Minority AIDS Initiative Continuum of Care program. S/he will collect, synthesize, and report all program data, program outcomes and evaluation to Quality Assurance and/or the Independent Evaluator. S/he will report directly to the Program Director.
Candidate must possess a Masters Degree in a Human Services Field, and have a minimum of 2 years of related experience.
