Hepatitis B Care Navigator , Philadelphia Department of Public Health, Division of Disease Control, Viral Hepatitis Program

The Hepatitis B Care Navigator will be a part of a project to eliminate Hepatitis B in Philadelphia, led by the Philadelphia Department of Public Health (PDPH). This project includes collaboration with two Federally Qualified Health Centers (FQHC) and other community-led organizations to improve prevention, testing, and care capacity in Philadelphia, along with a data-to-care approach. The Hepatitis B Care Navigator will work as a part of interdisciplinary teams within PDPH, at the two FQHCs, and with other care providers to ensure people living with hepatitis B infection are linked to hepatitis B-specific care and services needed to provide lasting hepatitis B care. Additionally, the Hepatitis B Navigators will provide education to people living with hepatitis B infection as well as collect data to inform the research and public health surveillance components of the elimination project. The Hepatitis B Care Navigators will work closely with the Project Team, as well as the viral hepatitis surveillance staff at PDPH and at partnering providers.\
