Hep C Patient Navigator / Outreach Worker
The Hepatitis C Outreach Worker/Patient Navigator works within Mount Sinai’s Primary Care Based Hepatitis C program, and the Hepatitis Education and Liver Screening Program” (HEALS)

The Navigator’s role is to assist patients in navigating the medical system to complete a medical evaluation and initiate hepatitis C treatment. The Navigator reinforces hepatitis C medical education with the patients and provides support to the patient throughout the process. The Navigator attends and schedules clinic visits with the patients and assists the clinical providers with managing the patients and clinic sessions. The Navigator is responsible for assisting with meeting data entry and reporting requirements for grant funding agencies.
The Navigator also participates in all HEALS community outreach activities. Outreach activities include educational presentations, hepatitis C rapid testing (including pre and post test counseling), and facilitating appointments in the clinic for those who test positive.
The Navigator is supervised by a Licensed Clinical Social Worker. This is an ideal position to gain experience in clinical medicine. Many individuals have gone on from this position to pursue advanced training in public health and medicine.
This is a grant funded position.
Specific responsibilities are below:
Position Description
Follow-up to clinic visits, including scheduling all appointments and follow-up on clinician referrals;
- Call patients to remind them about appointments
- Schedule work-up procedures (ultrasounds, etc)
- Obtain prior approval for procedures from patients’ insurance providers
- Assist patients with application for pharmaceutical company patient assistance programs
- Assist patients with applications for free care or with referrals to social worker
- Update the patient databases in AIRS and ACCESS
- Track patient follow-up, run queries
- Assist with reports to funding organizations or for grant writing
- Assist patients with other activities to decrease barriers to care
- Compose weekly email regarding upcoming clinical visits
- Provide feedback to HCV team during weekly meetings
- Take case conference notes on patients and send out to clinical providers
Attend the Hepatitis C (HCV) patients clinic visits during clinic sessions with patients.
Provide support for patients and address their questions and concerns;
- Clarify information and answer questions regarding disease pathology, treatment eligibility and evaluation, hepatitis C treatment and medications, and managing co-morbidities
- Assist patients on hepatitis C treatment by providing support for patients while on hepatitis C treatment, by providing assistance in managing side effects or other problems
- Attend and assist in coordinating monthly evening support group
Assist clinicians
- Update clinician on all new developments concerning patient
- Translate for Spanish speaking patients
- Order patient’s hepatitis C medication
- Track prescriptions and refills
Assist with research and grant activities as needed
- Literature review, recruitment, consenting patients, conducting interviews, providing follow-up to Project Manager, IRB training, etc
- Participate in clinic Quality Initiative activities
- Assist with peer program planning and implementation
- Attend Harlem HCV Task Force Meetings
Screening and Blood tests;
- Consent and pre and post test counseling
- Medical history intake
- Conduct hepatitis C rapid tests
- Option for phlebotomy training to conduct hepatitis C confirmatory blood tests
- Attend scheduled health fairs and outreach activities
- Make appointments for tests and results for follow-up care
- Coordinate with DOH (supplies, data entry, lab)
- Assist outreach coordinator with outreach event planning
Qualifications:
- Bachelor’s Degree required
- Spanish speaking preferred
- Database management experience
- Exceptionally organized and able to work independently
- Excellent communication skills
- Compassionate and personable
- Availability on nights and weekends required at times
- Willing to work in the field with community members
- 2 year commitment desired
- Interest in health disparities
- Phlebotomy skills a plus
Program Description
The Icahn School of Medicine at Mount Sinai, Division of General Internal Medicine, Research: The Division of General Internal Medicine is the largest division in the Department of Medicine and has a cadre of experienced grants managers, project managers, research nurses, recruiter/interviewers, and research assistants. Areas of research focus in the Division include chronic disease epidemiology and management, health services research, outcomes research, quality of care, health disparities, study of health beliefs and behaviors, cancer screening and treatment among minorities, clinical prediction rules, and doctor-patient communication.
