Patient Navigator – Substance Abuse Counselor

Care Coordination Program Patient Navigator and PATH Program Patient Navigator will provides adherence education and support to HIV/AIDS clients to ensure that they get connected to medical care and stay connected; will link clients to community resources. May assist with developing relations with collaborating agencies in order to identify and enroll in the PATH Program individuals with HIV/AIDS within the community who are not connected to ongoing primary care. Daily activities include, but are not limited to, patient accompaniment to appointments, field outreach, and administrative tasks.
Minimum Qualifications:
Applicants should have a strong knowledge of HIV/AIDS as well as entitlements and resources available to individuals living with HIV/AIDS in the NYC-metro area. In addition, applicants should be fluent in both English AND Spanish. Knowledge of Microsoft Word is a must. A Bachelor’s Degree in a health-related field is preferred.

Substance Abuse Counselor (SAC): ASCNYC/COACH (Comprehensive Outreach and Coordination to Health) Counselor must be knowledgeable of substance abuse issues, including harm reduction, recovery readiness, relapse prevention, and entering and maintaining substance abuse treatment, HIV, and viral hepatitis. The SAC is responsible for the substance abuse counseling, including but not limited to maintenance of referral center for regularly used substance abuse treatment or detox facilities. Experience in working with about reentry population is essential, as is experience with motivational interviewing, screening, assessment, service plan and re-assessment and GAIN Assessment tool.
Minimum Qualifications: The SAC will have a CASAC certificate from an accredited training institution, plus two years of experience or equivalent years of experience working with a range of treatment modalities. The SAC must also have experience working with people with HIV/AIDS, ex-offenders, harm reduction and alternative models of recovery, counseling and assessment skills, excellent interpersonal skills, and three to five years drug free preferred. Knowledge of Microsoft Word is a must. A Bachelor’s Degree in a health-related field is preferred.
