Position Summary:
The Hep C Care Coordination Nurse will provide care coordination for patients with Hep C infection who are in treatment for opioid use disorder at Greenwich House Center for Healing. The care coordination nurse will work with an interdisciplinary team comprised of on-site Hep C treatment providers, counselors, and a Hep C certified recovery peer advocate to engage patients in Hep C treatment and to educate patients about Hep C prevention, treatment, and harm reduction. Responsibilities also include engaging and supporting patients in treatment for Hep C, tracking patient progress in Hep C treatment, assisting the medical team with acquiring Hep C medications, providing and managing Hep A and B vaccinations, tracking cure rates and reinfections, and coordinating general medical care for patients engaged in Hep C treatment.
Primary Responsibilities:
- Educate patients about Hep C infection
- Manage and oversee implementation of annual Hep C screening of patients
- Facilitate referral to on-site medical providers for treatment of Hep C
- Track progress and cure rates of patients receiving Hep C treatment
- Provide Hep A, Hep B, and influenza vaccinations
- Manage vaccine ordering, tracking and documentation
- Assist in education of staff on Hep C infection and treatment
- Provide harm reduction counseling to patients with the goal of reducing overdose deaths, infectious disease, and Hep C reinfection
- Track population data on Hep C infection rate within clinic patient population
- Review laboratory tests
- Educate patients on Hep C transmission, treatment, prevention, and harm reduction in individual and group sessions
- Participate in regular meetings to review program progress, identify patient needs, and review infection and cure rates
- Develop protocols for prior authorizations and delivery of Hep C medication to clinic
- Collaborate with nursing team to provide directly observed Hep C treatment when indicated
- Facilitate referrals to specialty and primary care
- Work collaboratively with an interdisciplinary team of medical providers, directors, certified recovery peer advocates, and counselors to ensure maximal access to treatment and engagement of patients in Hep C treatment.
- Perform other tasks and special projects as assigned by the direct supervisor
Qualifications:
- Registered Nurse with New York State license
- Experience providing care for people with substance use disorders or Hep C preferred
- Skilled in establishing rapport with patients who have substance use disorders and other mental health conditions
- Embraces harm reduction as essential in treatment for Hep C and opioid use disorder
- Skilled in communication, organization, leadership, and clinical care
- Works well in interdisciplinary teams
Vaccination Requirement:
