Hep C Patient Navigator at Mount Sinai NYC

General Summary:

The Patient Navigator will be deeply involved in the entire spectrum of the Hepatitis C virus screening and linkage to care project as part of the Division of Liver Diseases. Overall responsibilities will include educating patients and providers on HCV screening, contacting patients to ensure linkage to care and maintaining a database.

Primary Goals of Navigator:

  • Assess effectiveness of HCV screening and linkage to care strategies for patients receiving primary care and inpatient care at Mount Sinai
  • Educate providers on HCV screening and new changes
  • Improve linkage to care for patients receiving care at Mount Sinai that have a positive hepatitis C screen

Duties and Responsibilities:

  • Assists with educational activities for primary care and inpatient providers related to HCV screening and follow-up care
  • Ensures appropriate patient education material is available in clinic and inpatient floors
  • Counsels inpatients with newly diagnosed HCV if assistance is requested by the inpatient service
  • Identifies patients in primary care clinics that would be eligible for HCV screening and approaches patients/ Medical Assistants during clinic hours to offer screening
  • Works with care coordinator to review monthly data reports on HCV screening and linkage to care rates and targets screening activities accordingly
  • Participates in dissemination of feedback reports to providers and nurses
  • Contacts patients with positive HCV screens that do not have appropriate follow-up care (viral load or referral to care) and provides appropriate referral to liver clinic
  • Assists select patients in following up with care, including transportation to appointments
  • Communicates effectively and professionally with patients, members of the health care team and project leadership
  • Participates in expansion of HCV screening efforts at additional health partners sites
  • Participates in pilot feasibility study for HCV screening in the Mount Sinai Hospital Emergency Department
  • Maintains linkage to care agreements with federally qualified health centers/ community health centers for undocumented and uninsured patients
  • Contributes to scholarship activities including oral and poster presentations and manuscript development

Requirements

  • Bilingual in Spanish is a plus (not required)
  • Motivated, friendly and personable
  • Full Time
  • Experience with Microsoft Excel and other computer software including EPIC
  • BA degree or higher
  • Excellent communication and telephone skills
