Project Description:

The REAH Program is a primary-care clinic for people who use drugs (PWUD) at Mount Sinai Hospital in New York City. Using a multidisciplinary team model, REACH provides harm reduction-focused primary care, medications for opioid and alcohol use disorder, and Hep C testing and treatment. The clinical team is committed to harm reduction and trauma-informed approaches to patient care that acknowledge systemic racial injustice and provide respectful, compassionate, and equitable care for all patients. The REACH Program also has a clinic at VOCAL-NY, a syringe service program in Brooklyn, to provide low-threshold medical care including Hep C testing and treatment to people who inject drugs.

The CHW’s role is to provide education and support to REACH Program clients at the VOCAL-NY clinic, provide Hep C rapid testing (including pre- and post-test counseling), and facilitate appointments in the VOCAL clinic for primary care and Hep C care and treatment. The CHW is also responsible for data and programmatic reporting to grant funding agencies.

The CHW is supervised by the Hep C Program Coordinator and the REACH Program Manager. The CHW is expected to spend approximately 60% of the time at VOCAL-NY in Brooklyn, 20% of the time at Mount Sinai Hospital in East Harlem, and the other 20% of the time has the option of working remotely.

Responsibilities:

  • Coordinate new clinic referrals
  • Provide education to (potential) patients about Hep C
  • Coordinate scheduling of appointments for patients
  • Document patient encounters
  • Track patient contacts
  • Assess patient barriers to accessing care and treatment
  • Conduct Hep C rapid tests
  • Enter programmatic data into AIDS Institute Reporting System (AIRS) data base
  • Prepare monthly reports to grant funding agency

Qualifications:

  • Bachelor’s Degree
  • Spanish speaking preferred
  • Exceptionally organized and able to work independently
  • Excellent communication skills
  • Compassionate and personable
  • Willing to work in the field with community outreach team
  • 2 year commitment desired
  • Interest in health disparities
  • Knowledge of harm reduction principles
  • Medical chart review experience
  • Data entry experience

Compensation:

The Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $60,000 – $72,000 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
