Job Description
Title: Patient Navigator – HIV and Hepatitis C
Location: CSEDNY – Queens
This position is a CDC/NYC Department of Health 5-year grant funded position. Within this position, the Patient Navigator will provide field-based services to clients enrolled in the program. The Patient Navigator will build strong relationships with the targeted population in order to provide leakage to care and to help them stay engaged in medical care, while adhering to medications, if needed. Patient Navigators are committed to removing the client’s barriers to care by identifying critical resources, helping them navigate systems, and promoting client health. The Patient Navigator will work closely with the medical doctor, counselors, and staff.
Specific Duties:
- Conduct community visits and outreach
- Provide linkage to HIV and HEP C care
- Provide Pre/Post education for HIV and HEP C
- Provide medical interpretation to client population
- Responsible for providing referrals and assistance in accessing supportive services
- Escort participants to medical appointments; assist in the navigation of health care systems; and provide any additional support requested, as needed
- Provide reminders of medical appointments (i.e. call, letters, text messages) as needed
- Provide medication/pharmacy assistance (i.e, supporting prior authorization or patient assistance program applications), as needed
- Monthly reporting to the RedCap Patient Navigation database
- Maintain tracking sheet and review with supervisor on a weekly basis
Qualifications:
- Bachelor Degree with at least two (2) years’ experience
- Certified by DOH for HIV and Hep C rapid testing
- Knowledge of HIV and Hep C exposure and treatment
- Good interpersonal skills
- Ability to deliver services to a diverse client population in an urban setting
- Availability to work with agencies in the community
- Must be proficient in MS word, Excel, and Data Entry
- Bilingual patient navigator (a plus)
