Post Tour Discussion
Callen-Lorde Services:
- Providing quality health care and related services for the lesbian, gay, bisexual, and transgender communities
- Providing rapid oral HIV/Hep C testing and linkage to care
- Offering primary care, mental health, oral health, and pharmacy services
- Open Monday-Saturday
- Currently they are not treating Hep C on-site. Instead, they link to external medical programs for treatment.
Discussion
- Discussion on providing Hep C referral and assistance with medication coverage at Callen Lorde. Group reviewed the internal processes for patients to receive assistance in linkage to care services that existed prior to the HEP TLC Coordinator contract & what occurs now. Now education, guidance & linkage is provided, which is beyond just referrals. For Hep C positive patients, the Hep TLC program handles the referral process and medication denials.
- PEP/PREP was mentioned as another service offered by Callen-Lorde department during our presentation of the Prevention & Outreach department.
- Callen-Lorde has over 30 Hep C positive patients in their program. More than 15 patients have been identified since the program began in April 2014.
- Praxis Housing gave a brief intro on their housing program
- They are the largest HIV housing program in NYC with housing in the Bronx, Manhattan and Brooklyn. Soon to be in Queens.
- Family Services Network of NYC provided info on their harm reduction services
- Providing Hep C outreach and referral services as well as needle exchange (Part of the IDUHA Hep C Peer Navigation Program)
- NYC Health Department provides Training:
