Job Summary
The Patient Navigator provides quality and comprehensive HCV testing and Health Education, Risk Reduction and HCV prevention education services targeting PWID at high risk for infection or transmission of HCV.
Core responsibilities include participant intake and assessment, coordination of referral services into appropriate programs, HCV testing, program outreach and promotion, safe sex kit distribution and referral/connection to high impact prevention activities. All program activities will address and empower each participant’s efforts to reduce the risk of HCV infection or transmission. Pt. Navigator also conducts HIV rapid testing and education/referrals. Must demonstrate a commitment to harm reduction theory and practice, commitment to advocacy and activism, particularly in the case of HIV/AIDS, hepatitis C, homelessness and drug policy.
Essential Duties and Responsibilities
• Provide excellent customer service at all times
• Conduct targeted HCV/HIV point-of-care testing and pre/post-test counseling during outreach and onsite at HRC
• Collect and organize oral swab and blood specimens (finger stick)
• Conduct confirmatory testing in collaboration with the NYCDOHMH
• Clearly and accurately document participant charts
• Ensure program deliverables are met annually
• Obtain all necessary releases to ensure case manager is aware of participants testing status and treatment needs
• Ensure that HIV and HCV positive participants are referred and connected to Medical Care
• Escort participants to appointments when approved by supervisor
• Able to lead and guide 1 Peer Navigators assigned to the program
• Participate in on-going trainings as needed
• Assist with the development and facilitation of educational HCV/HIV groups and support group(s)
• Establish relationships with multiple medical providers in the city
• Follow universal safety precautions at all times
• Attend and participate in local community events as needed
• Complete Accurate and timely submission of data reports, including data entry & submission of performance tracking reports
• Meet at least 90% of all monthly HCV/HIV testing related deliverables as outlined on the contract work-plan
• Attend monthly NYCDOH Check HEP C meetings
• Other duties as assigned by supervisor
Qualifications/Criteria
Associate degree or GED/HS diploma with 1+ years of relevant testing in HCV and HIV experience
Language: Bilingual- English and Spanish preferred.
Salary: $45,000.00 annually Full Time
Schedule: M-F _9-5pm
