General Summary:

The Project Coordinator will be deeply involved in the entire spectrum of the hepatitis C virus screening and linkage to care project as part of the Division of Liver Diseases’ STOMP-C initiative. Overall responsibilities will include maintaining a database of patient information and disseminating feedback to providers.

Primary Goals of Project Coordinator:

  • Maintain accurate and current data on HCV screening and linkage to care rates for patients receiving primary care and inpatient care at Mount Sinai
  • Communicate data effectively to healthcare providers and project leadership
  • Work with project leadership to improve HCV screening and linkage to care rates

Duties and Responsibilities:

  • Assists with educational activities for primary care and inpatient providers related to HCV screening and follow-up care
  • Oversees patient navigator(s) to ensure appropriate patient education material is available in clinic and inpatient floors
  • Reviews monthly data reports on HCV screening and linkage to care rates and organizes data by department, floor and provider when appropriate
  • Generates and disseminates data feedback reports on screening and linkage to care rates
  • Informs patient navigator(s) of patients with positive screens that have not been linked to care and follows-up to determine ultimate outcome of linkage to care attempts
  • Provides quarterly data feedback to Gilead Sciences, Inc while project is funded by FOCUS award
  • Communicates effectively and professionally with patients, members of the health care team and project leadership
  • Participates in expansion of HCV screening efforts at additional health partners sites
  • Participates in pilot feasibility study for HCV screening in the Mount Sinai Hospital Emergency Department
  • Maintains linkage to care agreements with federally qualified health centers/ community health centers for undocumented and uninsured patients
  • Contributes to scholarship activities including oral and poster presentations and manuscript development

Requirements

  • Full Time
  • Experience with Microsoft Excel and statistical analysis software (SAS, STATA or SPSS)
  • Masters in Public Health or equivalent
  • Excellent communication and telephone skills
