The Patient Navigation Coordinator is a full-time staff member in the Mount Sinai Respectful & Equitable Access to Comprehensive Healthcare (REACH) Program in the Division of General Internal Medicine of the Department of Medicine, Icahn School of Medicine at Mount Sinai. The REACH Program is a primary-care clinic for people who use drugs (PWUD) at Mount Sinai Hospital in New York City. Using a multidisciplinary team model, REACH provides harm reduction-focused primary care, medications for opioid and alcohol use disorder, and Hepatitis C Virus (HCV) testing and treatment. The clinical team is committed to harm reduction and trauma-informed approaches to patient care that acknowledge systemic racial injustice and provide respectful, compassionate, and equitable care for all patients.

The Navigation Coordinator’s role is to assist patients in navigating the medical system and provide them with support as well as to support the patient navigation team. The Navigator attends and schedules clinic visits with the patients and assists the clinical providers with managing the patients and clinic sessions (whether telehealth visits or in-person visits). The Navigator is responsible for assisting with meeting data entry and reporting requirements for grant funding agencies.

The Navigation Coordinator also participates in REACH community outreach activities. Outreach activities include educational presentations, hepatitis C rapid testing (including pre- and posttest counseling), naloxone kit distribution, and facilitating appointments in the clinic for primary care and HCV care and treatment.

The Navigation Coordinator is supervised by a Licensed Clinical Psychologist and the Program Manager.

Specific responsibilities are below:
Follow-up to clinic visits, including scheduling all appointments and follow-up on clinician
referrals;

  • Call patients to remind them of appointments
  • Schedule work-up procedures (Ultrasounds, etc.)
  • Obtain prior approval for procedures from patients’ insurance providers
  • Assist patients with application for pharmaceutical company patient assistance programs
  • Update the patient databases in AIRS and ACCESS
  • Track patient follow-up, run queries
  • Assist with reports to funding organizations or for grant writing
  • Assist patients with other activities to decrease barriers to care
  • Compose weekly email regarding upcoming clinical visits
  • Provide feedback to the HCV team during weekly meetings
  • Take case conference notes on patients and send out to clinical provider

Attend the patient’s clinic visits during clinic sessions with patients.
Provide support for patients and address their questions and concerns;

  • Clarify information and answer questions regarding disease pathology, treatment eligibility and evaluation, hepatitis c treatment and medications, and managing comorbidities
  • Assist patients on hepatitis C treatment by providing support for patients while on hepatitis C treatment, by providing assistance in managing side effects or other problems
  • Assist in coordinating REACH Program support groups

Assist clinicians;

  • Update clinician on all new developments concerning patients
  •  Translate for Spanish speaking patients
  • Order patients hepatitis C medication
  •  Track prescriptions and refills

Support patient navigation team;

  • Monitor and coordinate work responsibilities across patient navigation team
  • Provide ongoing skills training to other patient navigators
  • Serve as supportive resource for other patient navigators
  • Advocate for patient navigation staff needs to program leadership

Assist with research and grant activities as needed;

  • Literature review, recruitment, consenting patients, conducting interviews, providing follow-up to Project Manager, IRB training, etc.
  • Performing chart abstractions and entering data into REDCap and ACCESS
  • Participate in Quality Initiative activities
  • Assist with peer program planning and implementation
  • Attend NYC HCV Task Force Meetings

Screening and Blood tests;

  • Consent and pre- and post-test counseling
  • Medical history intake
  • Conduct hepatitis C rapid tests
  • Attend scheduled health fairs and outreach activities
  • Make appointments for tests and results for follow-up care
  • Coordinate with DOH (supplies, data entry, lab)
  • Assist outreach coordinator with outreach event planning

Qualifications:

  • Master’s Degree preferred
  • Spanish speaking preferred
  • Database management experience
  • Exceptionally organized and able to work independently
  • Excellent communication skills
  • Compassionate and personable
  • Occasional availability on nights and weekends at required times
  • Willing to work in the field with community members
  •  2 year commitment desired
  • Interest in health disparities
  • Interest in working with patients with substance use disorders/psychiatric disorders
