Position Summary
Working with the PCMH care team, including the medical provider, nurse coordinator, medical case manager, and other support staff, the PEP/PrEP/Hep C Navigation Coordinator is responsible for the coordination and support of PEP, PrEP, and Hepatitis C patients through the screening, assessment, care planning, and treatment processes. They are responsible for implementation and administration of internal and PHS/DOHMH contracted Hep C, PEP, and PrEP programs.
