Patient Services Coordinator at Hep C Connection

Summary of position:

Plan, implement, and track activities related to Hep C Connection’s HelpLine, Patient Navigator program, and community outreach events.  Additionally, assist with fundraising and awareness activities.

Essential Duties and Responsibilities:

Manage the HelpLine:

  • Answer and return calls using national databases and the internet to research questions, resources and support for callers
  • Collect information from callers to log into the program database
  • Maintain database software and pull reports as needed
  • Train for and take on shifts answering calls through the national hepatitis C hotline, Help 4 Hep
  • Maintain a training manual and provide updates as necessary

Manage the Patient Navigator Program:

  • Schedule and conduct testing and patient navigator services
  • Provide education, testing and risk reduction counseling to high-risk individuals
    at initial appointment and follow-up appointments with goal of linking client to treatment
  • Track HCV testing and patient navigator outcomes in Microsoft Excel; assist Director of Community Outreach with monthly testing report
  • Maintain a training manual and provide updates as necessary
  • Be the liaison between Hep C Connection and Rocky Mountain Cares

Manage Marketing and Social Media Tools:

  • Create content and update the website using WordPress
  • Create and send out a monthly electronic newsletter, as well as periodic mailings
  • Maintain our Facebook and YouTube accounts
  • Update Hep C Connection’s online profile on various business/review sites
  • Recruit e-newsletter subscribers

Coordinate Volunteers and Outreach Events:

  • Recruit, train, schedule and supervise volunteers and interns for the HelpLine, outreach events, fundraising events, and office projects
  • Organize staffing and prepare materials for health fairs

Assist with Fundraising Efforts:

  • Serve as an integral committee member for our annual fundraising event, Desserts & Delights
  • Represent Hep C Connection with Community Health Charities
  • Enter and pull data into/from the donor database for mailings

Other duties as assigned.

Job Qualifications:

  • Bachelor’s degree from an accredited institution required (preferably in a human services field); master’s candidate in social work, public health or counseling preferred
  • Good listening, questioning and organizing skills
  • Positive attitude, enthusiastic team player, self-starter who will fulfill the mission of Hep C Connection
  • Experience or knowledge of hepatitis and/or chronic disease management preferred
  • Proficiency with Microsoft Office, FileMaker Pro and WordPress
  • Ability to learn and retain information, follow instruction and make educated decisions
  • Reliable transportation
  • Certification or training on HIPAA compliancy
  • Ability to provide finger pokes with lancets to clients while adhering to universal precautions
  • Bilingual proficiency speaking Spanish a plus
  • Training on cultural competency, universal precautions, motivational interviewing, and HIV risk-reduction counseling and testing a plus.  However trainings will be provided if needed.

Work Schedule:  Full-time, Monday through Friday with occasional evening and weekend work required.

Compensation:  Salary commensurate with experience, employer-paid health and dental insurance, and covered garage parking.
