POSITION: SOCIAL WORKER (PART-TIME)

DESCRIPTION:

The social worker in this part-time position (3 days/week) will be in the Respectful & Equitable Access to Comprehensive Healthcare (REACH) Program at IMA in the CAM Building IMA Practice on 102nd Street. There is some flexibility in determining the three days per week.

The REACH Program offers primary care services to patients who use alcohol and/or drugs or who are in recovery from addiction. Using a harm reduction model, REACH offers overdose prevention services, access to buprenorphine and other medicines to treat substance use disorders, and comprehensive primary care that integrates behavioral health (psychology in addition to social work). The REACH program further offers care coordination support from patient navigators and services to address psychosocial barriers to health. The REACH Program began as an outreach, testing and treatment program for patients with Hepatitis C Virus (HCV) infection and continues to offer these services as well, within the primary care and outreach settings.

The social worker will be present at REACH clinic sessions where he/she will work directly with patients to provide crisis intervention, short-term counseling, support in adjustment to buprenorphine treatment, and referral for mental health, substance abuse treatment and benefits/entitlements. He/she/they will contribute to creating a warm and welcoming clinic environment, provide support to providers about clinical harm reduction best practices and contribute to a team-based approach to problem solve systemic issues as they arise. He/she/they will have the opportunity to co-lead support groups, co-lead the REACH Racial Justice Initiative, conduct outreach and education in the community to build and strengthen partnerships, and participate in research initiatives to improve understanding and care of this vulnerable and diverse population of patients. Responsibilities include tracking all referrals and follow-up with patients and referral contacts, documentation in the medical record, coordination of care with the medical staff and patient navigators, and participation in weekly case conferencing. The social worker will also provide support and consultation to the care coordinators of the Liver Education & Action Program (LEAP) in the Division of Liver Diseases.

REQUIREMENTS:

  • Experience with adults with chronic illness.
  • Experience with adults with substance use disorders and harm reduction approach.
  • Relevant experience with psychosocial assessments and counseling.
  • Training in Trauma Informed Care and Motivational Interviewing.
  • Exceptional collaborative, clinical, and organizational skills.
  • Ability to work independently.
  • Bilingual (English/Spanish) strongly preferred.
