Job Description:

The Bedford-Stuyvesant Family Health Center (BSFHC), a patient-centered, team based multi-specialty practice in central Brooklyn, has been providing affordable, comprehensive, outpatient medical and dental services in an outpatient setting for over 40 years. As a Federally Qualified Health Center (FQHC), BSFHC serves all of the primary health care needs of families in the heart of North and Central Brooklyn. Our mission is to provide the most professional, courteous and highest quality health care, with dignity, to those we serve, especially the undeserved population, without regard for ability to pay.

We are seeking HIV and Hep C Patient Navigator. The Patient Navigator role is crucial in ensuring that patients receive seamless, comprehensive care by guiding them through the healthcare system and connecting them with the necessary services and resources. The Patient Navigator will work closely with clients, healthcare providers, and community organizations to improve healthcare access, particularly for underserved populations, including people living with Hep C and HIV and those requiring behavioral health and chronic disease management services.

** Position is grant-funded: Contingent upon continuation of grant funding from year to year.

Responsibilities:

  1. Serve as a primary point of contact for Hep-C patients, guiding them through the healthcare system and ensuring they access the services they need.
  2. Assist patients in understanding their diagnosis, treatment options, and care plans, and help them make informed decisions.
  3. Coordinate appointments, follow-ups, and referrals for patients, ensuring timely access to care and services.
  4. Provide support for patients in overcoming barriers to care, including transportation, insurance issues, and language barriers.
  5. Collaborate with healthcare providers, social workers, and community organizations to create and implement comprehensive care plans for patients.
  6. Conduct outreach to identify and engage underserved and out-of-care individuals, especially PLWH, and connect them to healthcare services.
  7. Assist patients in enrolling in insurance programs and other financial assistance programs to help cover the costs of their care.
  8. Maintain detailed records of patient interactions, care plans, and progress, ensuring accurate data collection and reporting.
  9. Educate patients on health maintenance, medication adherence, and chronic disease management.
  10. Participate in community outreach activities, including health fairs and educational workshops, to promote awareness of available healthcare services.
  11. Support the implementation of BSFHC’s Access Health Initiative by connecting community members to services and resources.
  12. Perform additional duties as requested by the Supervisor.

Qualifications:

  • Bachelor’s degree in a related field such as Social Work, Public Health, Nursing, or Health Education
  • In-depth understanding of community cultures, ethnicities, and local resources to address healthcare challenges effectively
  • Familiarity with Brooklyn-based healthcare systems is essential
  • Strong organizational skills with the ability to manage multiple tasks and prioritize goals effectively
  • Excellent communication skills, both verbal and written, with the ability to build positive relationships with diverse populations, including underserved and hard-to-reach groups
  • Ability to work independently as well as in a team environment
  • Experience working with populations affected by HIV/AIDS, LGBTQ communities, and communities of color
  • Demonstrated ability to provide culturally and linguistically appropriate care
  • Proficiency in documenting patient interactions and maintaining accurate records
