Apicha Community Health Center is seeking a Care Coordinator to provide navigation services to HIV/AIDS who are virally unsuppressed, individuals with Serious Mental Illness, serious substance use disorders (SUDs), and/or chronic hepatitis.
The Patient Navigator will build strong relationships with the targeted population in order to provide leakage to care and to help them stay engaged in medical care, while adhering to medications, if needed.
The Case Manager/Counselor will provide supportive counseling services; facilitate continued engagement in medical care & adherence to treatment to HIV positive individuals; will also provide supportive services to non-HIV individuals.
