The major duties of the position include, but are not limited to:
Leadership –
- Lead a team of patient navigators and patient assistant to ensure care coordination services are provided to all clients assigned to meet clients’ needs for better health outcome.
- Provide day-to-day supervision to the patient navigators and patient assistant assigned by weekly schedule monitoring, chart review, face-to-face case discussion, and performance reviews.
- Train and coach the assigned staff and/or new team members of workflow and requirement changes), and performance expectations and excel in their performance.
- Responsible for ensuring the team meets services quota established and contractual requirement; if circumstances cause changes, plan and implement to meet target goal.
Client Services –
- Responsible for implementation of service/care plan, supported by Patient Navigators.
- Provides clinic-based health education in conjunction with regularly schedules primary care visits.
- Performs the initial entitlements assessment and collates the comprehensive plan.
- Facilitates interdisciplinary conversation and planning.
- Working as liaison to medical provider with the implementation of client service plan, supported by Patient Navigators
- Collaborates with the clinical team to manage care coordination resources and target the neediest PLWHA for service.
- Performs intake screening and assessment for program enrollment including determination of level of services need, care planning, care conference, etc.
- Served as back-up for the assigned staff as needed
Collaboration and Services –
- Conduct outreach & recruitment for Apicha CHC to promote the Agency services.
- Filed and response to telephone, e-mail, and other requests for information.
- Provide information and referral inquiries of services available at the Agency.
- Document, track, and monitor services provided. Maintain accurate records of services rendered.
- Ensure the program staff and clients conform to rules and regulations as required.
- Collate data reports (e.g., appointment dispositions, laboratory results, etc.) produced by clinic information systems.
- Point of contact for hepatitis and HIV related services.
Administrative and Supervisory –
- Created and maintaining up to date case supervision logs.
- Maintain accurate records on the services and activities such as attendance, registration, client’s interactions and other required documentation according to the protocols and standards of APICHA’s Department of Support Services in designate database and reporting structures within required timeframe
- Perform quality assurance and quality improvement activities.
- Participates internal and external meetings including assists as requested and appropriate.
- Adheres to the protocols and standards of Apicha’ s Department of Support Services that inclusive of DOHMH Care Coordination program.
- Working as a part of the team for the quality assurance and continue quality improvement (CQI) tasks.
- Ensure all service requirement under assigned team are met.
Qualifications:
- BA/BS in related field with two (2) years of qualifying experiences required. An advanced degree in one of the qualifying fields may be substituted for one (1) year of qualifying experience may be considered.
- Experience working with target populations strongly preferred. The target populations: HIV/AIDS who are virally unsuppressed, individuals with Serious Mental Illness, serious substance use disorders (SUDs), and/or chronic hepatitis.
- Ability to adapt changing environment
- Appreciation of and commitment to Apicha’s mission and values required
- Excellent MS Office skills (Outlook, Word, Excel, PowerPoint) required.
- Ability to work effectively in both a collaborative as well as individual setting.
- Must possess ability to interact with all levels of external and internal customers with a high level of professionalism and tact in representing Apicha CHC.
- Excellent customer service skills to assist in resolving member complaints.
- Strong verbal and written communication skills – for both internal and external audience.
