Care Coordinator Supervisor at SUNY Downstate
SUNY Downstate Medical Center is seeking a Care Coordinator to supervise a team of 4 Patient Navigators. Excellent opportunity to join an innovative program that improves health disparities by advancing key elements of the CDC’s HIV Continuum of Care. Performance-based outcomes include retention in care, medication adherence and viral load suppression among HIV clinic patients.
Requirements:
BA/BS or LMSW/LCSW/LMHC or RN/LPN degree with at least 2 years of case management experience and 2 years supervisory or program management experience. Bilingual in English/Haitian Creole or English/Spanish preferred.
Ideal candidate has excellent verbal/written communication skills, is pro-active, very organized, and has proven record of achieving time-sensitive grant objectives.
Experience working with populations who are living with HIV, substance use and/or co-occurring disorders preferred. Bilingual in English/Haitian Creole or English/Spanish preferred. Experience with Electronic Medical Record (EMR) preferred.
