Check Hep C Patient Navigation Program
The goal of the Check Hep C Patient Navigation Program is to link NYC residents living with hepatitis C (Hep C) to medical care. Patient Navigators support patients to complete Hep C medical evaluation and successful Hep C treatment, prevent re-infection, and maintain liver health after treatment. This program is funded by New York City Council and private institutions, and is administered by the NYC Health Department.
