Project INSPIRE: NYC Hep C Care Coordination Program

Project INSPIRE is a comprehensive hepatitis C care coordination program in NYC. New York City residents diagnosed with hepatitis C who have Medicaid or Medicare can enroll in the program. The program offers patient navigation and clinical care coordination services to support patients through the hepatitis C care and treatment. Project INSPIRE is now enrolling through February 2017. This program is made possible with funds from Centers for Medicare & Medicaid Services.
