Position Summary: The Hepatitis C Program Manager is responsible for the implementation, management, and administration of an AIDS Institute–funded Hepatitis C treatment program. This position works collaboratively with a care team that includes the medical provider, nurse coordinator, medical case manager, Hep C Navigator, and additional support and administrative staff. The Program Manager will be located at the East New York Community Health Center and responsible for program implementation at two Brooklyn–based sites. It reports to the Director of PrEP and Hepatitis C Initiatives.

Program components include managing a team responsible for targeted outreach and linkage to care, peer-delivered services, care coordination, building community partnerships to increase client engagement, data entry and reporting, and supporting clients on the path of hepatitis C care and treatment, utilizing a multidisciplinary care team approach in a primary care setting. Team members work with a complex population that includes people who use drugs, people experiencing housing instability/homelessness, people with mental health issues, and others experiencing social determinants of health. In addition to primary care, this position will collaborate with other Housing Works programs such as Harm Reduction and OASAS Substance Use Treatment.

Essential Functions of Position:

1. Ensure effective day-to-day management of contract activities

2. Directly supervise the Hep C Navigators and Peer Outreach Workers

3. Collaborate with clinic and program staff to ensure effective communication

4. Track and report on progress toward meeting contract deliverables

5. Liaise with funding organizations

6. Ensure high-quality client experience by ensuring all client care is coordinated across Housing Works programs and addressing client issues and barriers to care as necessary

7. Promote contract goals and participation across internal Housing Works programming

8. Track and report on progress toward meeting contract deliverables

Required Knowledge, Skills, and Abilities:

1. Demonstrated ability to work effectively independently, manage a team, and collaborate across departments, required

2. Excellent written and verbal communication skills required

3. Commitment to maintaining highest levels of confidentiality

4. Demonstrated ability to build the trust and respect of patients, staff, colleagues, external contacts, and other Housing Works members

5. Sensitivity to needs of culturally and linguistically diverse patient and employee population

6. Demonstrated ability to meet or exceed the Service Excellence Standards of Housing Works

7. Willingness and ability to work some evenings and weekends, as needed

8. Demonstrated ability to utilize electronic medical records (EMRs) and related computer software, and willingness to adapt to the evolving technological requirements of modern health center practices

Required Academic and Experience Qualifications:

1. Bachelor’s degree required, master’s degree preferred (e.g., MSW, MPH)

2. Minimum three years of supervisory experience in project or program management, preferably in medically needy and underserved communities and in a community health center setting

3. Previous experience providing services and exercising leadership in a culturally and linguistically diverse setting, with demonstrated success

4. Previous experience working within a managed care environment strongly preferred

5. Previous experience working with substance use population

6. Previous experience building partnerships with community-based organizations preferred

7. Bilingual English/Spanish preferred, but not required
