As a member of a multidisciplinary team the Hepatitis C Intensive Case Manager (ICM) provides active and ongoing outreach, education, counseling, and intensive case management to people actively using drugs in order to facilitate initiation and successful completion of hepatitis C antiviral treatment. These responsibilities will be executed as part of a research study examining the effectiveness of delivering hepatitis C antiviral treatment to people actively using drugs using a multidisciplinary model of care structured around a collaborative partnership between SUNY Downstate Medical Center, Weill Cornell Medical College, and community-based needle exchange programs (NEPs). The ICM crosses institutional boundaries to deliver coordinated care onsite at partnering NEPs and the medical clinic.
