Check Hep C Patient Navigation Program

Check Hep C New York CityThe goal of the Check Hep C Patient Navigation Program is to link NYC residents living with hepatitis C (Hep C) to medical care. Patient Navigators support patients to complete Hep C medical evaluation and successful Hep C treatment, prevent re-infection, and maintain liver health after treatment. This program is funded by New York City Council and private institutions, and is administered by the NYC Health Department.

PROGRAM SERVICES

Check Hep C Patient Navigators provide the following services at no cost to the patient: Hep C health promotion, referrals to supportive services, linkage to Hep C medical care, accompaniment to or reminders for medical appointments, alcohol and drug counseling, case conferencing, treatment readiness and adherence support, and medication access support.

To be eligible, patients must reside in NYC and have a positive hepatitis C RNA test result.
Note: Hep C Antibody positive patients not tested for Hep C RNA are accepted at select syringe exchange programs.
