Hepatitis C Social Worker — Part Time — Mount Sinai

POSITION:

SOCIAL WORKER (Approx 6 hours/week, M & Tu 1 – 4 PM)

SERVICE:

Primary Care Hepatitis C (HCV) Program Internal Medicine Associates (IMA)

DESCRIPTION:

The social worker in this new, grant-funded, part-time position will be in the Primary Care Hepatitis C (HCV) Program at Internal Medicine Associates on 102nd Street. Current funding extends to September, 2012, and there is a possibility of continuation and, in the future, of additional hours. All patients in this Program have chronic HCV but are not co-infected with HIV. Patients often need assistance to be able to start and stay on the HCV treatment medications, which can be difficult to tolerate. Patients are mostly insured by Medicaid; many have a history of injection drug use and mental health issues. The social worker will be on-site at the Program’s two clinic sessions per week (Monday and Tuesday afternoons, 1 – 4 PM).

Responsibilities include conducting the Psychosocial Readiness Assessment Tool (identifying barriers to treatment), providing referrals and recommendations in collaboration with the Clinical Psychologist; providing individual counseling; creating and maintaining a resource guide for referrals; create tracking form and track all referrals and follow-up with patients and referral contacts; and provide assistance with health insurance and medication applications and other entitlements. The social worker will coordinate care with the medical staff and patient navigator and participate in case conferencing as needed. All efforts will be documented, with feedback provided to the HCV team as needed.

REQUIREMENTS:

Experience with adults with chronic illness. Experience with substance abuse preferred. Exceptional collaborative, clinical, and organizational skills. Ability to work independently. LCSW preferred.
